BACKGROUND This is Mr. Stephens' fourth application for workers' compensation benefits arising out of alleged industrial accidents at Larry Miller Dodge on September 8, 1986 and December 31, 1986. Mr. Stephens' previous claims have been dismissed for lack of supporting medical documentation. The ALJ likewise dismissed this claim for lack of medical documentation.
