The Industrial Commission of Utah exercises jurisdiction over this Motion for Review pursuant to Utah Code Ann. '63-46b-12, Utah Code Ann. '35-1-82.53, and Utah Admin. Code R568-1-4.M. BACKGROUND This is Mr. Stephens' fourth application for workers' compensation benefits arising out of alleged industrial accidents at Larry Miller Dodge on September 8, 1986 and December 31, 1986. Mr. Stephens' previous claims have been dismissed for lack of supporting medical documentation. The ALJ likewise dismissed this claim for lack of medical documentation.
