motion requesting further findings of fact and conclusions of
law and an appropriate decision if it differs from the
original. Within 15 days after filing the motion, the party
shall file its proposed findings of fact and conclusions of
law pursuant to 39-A M.R.S.A. §218 and 90 M.A.R. 351 Ch.
12 § 5.
Within

may request an appeal to the Appellate Division by filing a
notice of intent to appeal along with a copy of this decision

1. The
Employee injured his left knee at work when he fell getting
down from the trailer of a truck. He was diagnosed with a
left lateral tibial plateau fracture. His Average Weekly Wage
at the time of the injury was $614,31. He was paid lost time
