          DOIS: 03/16/2017,03/21/2018          Within 25 days after issuing this decision, or 5 days after ruling on a motion for further findings, whichever is later, the hearing officer may request that the full Workers' Compensation Board review this decision. See 39-A M.R.S. §320. Within 20 days after receiving this decision a party may file a motion asking the Board to find further facts and make further conclusions of law and file the appropriate decision if it differs from the original decision; within 15 days after filing the motion, the party shall file its proposed findings of tact and conclusions of law pursuant to §318 and 90 M.A.R. 351 Ch.12, §5. Any party in interest may request an appeal to the Appellate Division by filing a notice of intent to appeal along with a copy of this decision with 1he clerk of the Appellate Division within 20 (twenty) days of receipt of this decision. See 39-A M.R.S. §321-B.          The appeal rights set forth on this decree represent the Board's opinion as to the proper venue for appeal. To the extent there is a dispute as to the application of 39-A MR.S. §321-B to this decision, the opinion of the Board will not be controlling. Parties ate responsible for ensuring their appeals are filed in the appropriate forum. Failure to file in the appropriate forum may result in loss of opportunity for appellate review. See, Guaranty Fund Mgmt. Sens. V. Workers' Compensation Bd. 678 A.2d 578,582 (Me. 1986).          PURSUANT TO BOARD RULE CHAPTER 12 §19, ALL EVIDENCE AND TRANSCRIPTS IN THIS MATTER WILL BE DESTROYED IN 60 DAYS UNLESS (1) WE RECEIVE WRITTEN NOTIFICATION THAT ONE OR BOTH PARTIES WISH TO HAVE THEIR EXHIBITS RETURNED TO THEM OR (2) A PETITION FOR APPELLATE REVIEW TS FILED.
