STATEMENT
OF CLAIM:
Plaintiff
filed an Application for Hearing which was received by the
Bureau on June 25, 2004. She alleges on miscellaneous dates
she suffered an injury and disability as follows:
“Repetitive use of upper extremities caused, aggravated
or accelerated disability to right shoulder, hand and
bilateral upper extremities. Plaintiff was paid
Workers’ Compensation for these injuries for time
missed in the past. Plaintiff was working in a restricted
capacity for less than the one hundred weeks when she was
taken out of work for a stroke.”
STIPULATIONS:
The
parties executed a stipulation sheet which has been admitted
as Joint Exhibit 5. The parties have also agreed to a
stipulated state of facts setting forth the claim, a copy of
