STINE, JUDGE.          This matter came on for a Pre-Trial Conference pursuant to this Court’s October 2, 2019, Order. Prior to the beginning of the Pre-Trial Conference the parties mutually agreed to submit a Joint Pretrial Memorandum in lieu of discussing and stipulating to the matters set forth in the October 2, 2019, Order. Accordingly, it is hereby ordered that the following pretrial order shall govern all subsequent proceedings in this matter:          I.          On or before January 17, 2020, counsel for all parties shall file with the Clerk of the Workers’ Compensation Court a joint pretrial memorandum, which shall separately set forth the following:
A. Nature of the case (scheduled member; non-scheduled loss of earning power); brief description of occurrence of the accident; whether a counterclaim is asserted;
B. Stipulations of fact, such as employment, average weekly wage, etc.;
C. Issues for determination by the Court at trial (factual and legal issues). Issues not set forth in paragraph C will be deemed waived;
D. If payment of indemnity benefits is claimed in paragraph C, above, the parties shall set forth:
1. The dates, amounts, and types (TTD, PPD, TPD, etc.) of indemnity claimed, and the total number of weeks for which indemnity is claimed; and,
2. The dates, amounts, and types (TTD, PPD, TPD, etc.) of indemnity paid, and the total number of weeks of each type of indemnity paid;
E. If payment of medical expenses is claimed in paragraph C, above, the parties shall attach to the joint pretrial memorandum a medical expense cover sheet setting forth an itemization of each medical expense incurred and unpaid, or for which reimbursement is claimed, by provider, date, and amount;
