BLOCK, JUDGE          This cause came on for trial before the Nebraska Workers’ Compensation Court at Kearney, Buffalo County, Nebraska on December 21, 2017, on the Petition of the plaintiff, Answer of the defendant, and on the evidence, Judge Dirk V. Block, one of the judges of said court presiding. Plaintiff appeared in person and was represented by counsel. Defendant was represented by counsel. The Court accepted the following Stipulations of the parties:
1. That on February 2, 2015, plaintiff was an employee of the defendant;
2. That plaintiff had an average weekly wage of $297.00 for purposes of temporary and permanent disability;
3. That venue is proper in Buffalo County, Nebraska;
4. That past medical expenses incurred by the plaintiff, if awarded by the Court, may be paid pursuant to the Workers’ Compensation Fee Schedule.
         Testimony was taken, evidence adduced, cause submitted, and the Court being fully advised in the premises finds as follows:          I. ISSUES          ACCIDENT: whether the plaintiff had an accident arising out of and in the course of his employment with the defendant;          NOTICE: whether the defendant was given notice of injury as soon as practicable;          CAUSATION: whether the plaintiff suffered injuries caused by the accident at work;          DISABILITY: whether the plaintiff incurred periods of disability as a result of accident and injury while working for the defendant, and the extent and duration thereof;          MEDICAL CARE: whether the injuries suffered by the plaintiff necessitated medical care, past and future, and whether the treatment and corresponding bills previously incurred were as a result of the accident and injury;          VOCATIONAL REHABILITATION: whether the plaintiff has permanent injury...
