BLACK, ADMINISTRATIVE LAW JUDGE          STATEMENT OF THE CASE          A hearing was held on April 30, 2019, regarding this claim pursuant to Dillard v. Benton County Sheriff’s Office, 87 Ark.App. 379, 192 S.W.3d 287 (2004), to determine whether the above-referenced matter should be dismissed for failure to prosecute under Ark. Code Ann. §11-9-702(a)(4), and Rule 099.13 of the Arkansas Workers’ Compensation Commission.          The record consists of the transcript of the April 30, 2019, hearing and the documents contained therein. The entire Commission’s file has also been made a part of the record. It is incorporated herein by reference.          DISCUSSION          The claimant filed a Form AR-C with the Commission on or about April 14, 2015, alleging an injury to her spine after a lifting incident. She alleged that she sustained a work-related injury on November 17, 2014, while in the course and scope of her employment with the respondent-employer. The claimant checked all of the boxes for both initial and additional benefits. According to the Form AR-C, at the time of her alleged injury, the claimant was switching offices with a coworker, when she tried to lift a desk off of the computers and cords so they could continue to work.          The respondent-insurance-carrier controverted this claim in its entirety on February 10, 2015, pursuant to the Form AR-2.
