JAY QUIST, MAGISTRATE #192G, JUDGE          CASE SUMMARY          On September 27, 2004, plaintiff filed an Application for Mediation or Hearing -Form A. Plaintiff alleged that he suffered a crush injury to his left foot on June 25, 2004, during the course of his employment with Defendant Rock Tenn. Company.          The case proceeded to trial in Grand Rapids on May 4, 2006. The sole issue is whether plaintiff suffered a work-related injury to his left foot. Based on the evidence presented, I find that the plaintiff established a work-related injury.          STIPULATIONS          June 25, 2004 Injury Date          The parties stipulated that:          1. They were subject to the Workers' Disability Compensation Act.          2. Defendant Rock Tenn. was insured by Travelers Indemnity Company of Connecticut.          3. The plaintiff was in the employ of the defendant at the time of the alleged personal injury.          4. Defendant received timely notice of the plaintiff's alleged personal injury.          5.
