Defendant file their own Petition For Hearing – Form C, received March 28, 2006, seeking a determination that Plaintiff had not sustained a work-related injury or work disability. In an Order mailed April 25, 2006, Defendant’s Motion to Suspend and Forfeit Compensation for Plaintiff’s failure to attend an independent medical examination was granted. In an order mailed the same day, Plaintiff’s Petition for Hearing was dismissed.          Plaintiff filed another Application for Hearing, received June 8, 2006. Defendant subsequently again filed a Form C, received July 7, 2006, again seeking a determination that Plaintiff did not sustain a work-related injury or work disability.
