Interlocutory
Order
The
commission affirms the decision of the
administrative law judge (ALJ) subject to the modifications
set forth below. The self-insured employer, Mayo Clinic
HS-Oakridge, Inc., shall pay to the applicant the
compensation ordered to be paid to him in the ALJ's
Interlocutory Order; and to applicant's attorney, Thomas
Siedow, the fees and costs ordered to be paid to him in the
ALJ's Interlocutory Order.
Mayo
Clinic HS-Oakridge, Inc. is also prospectively liable for the
recommended treatment of a trial spinal cord stimulation.
Jurisdiction
is reserved for such further findings and orders as may be
necessary consistent with the findings of this decision.
Accordingly, within 30 days from this date.
Modifications
In the
first sentences of the first paragraph on page 3 of the
ALJ's decision, substitute "July 15, 2015" for
"July 1, 2015" and in the first sentence of the
third paragraph on page 3 substitute "May 31, 2016"
for "May 11, 2015".
The
rest and remainder of the ALJ's findings are affirmed and
reiterated as if set forth herein.

Procedural
Posture
On
March 6, 2017 the applicant submitted a hearing application
claiming permanent partial disability and the need for
additional treatment with spinal cord stimulation for
injuries arising of and in the course of her employment with
the employer. The date of the claimed work injury is November
22, 2013. On October 23, 2018 an ALJ for the Department of
Administration, Division of Hearings and Appeals, Office of
Worker's Compensation Hearings...
