On and after the passage of this act, no building shall be occupied or permitted to be occupied as a rooming-house or tenement-house unless the owner or lessee of such building, or the conductor of such tenement, shall have first made application to the Bureau or Board of Health for a license therefor. Such application shall be made in writing, and shall state the nature and extent of the applicant's interest in the property, the name and address of the owner or his agent, the location of the building, the use to which it is to be put, and the number of rooms to be occupied.
