On November 2, 2018, Claimant filed a Form AR-C, in which he alleged that he injured his shoulders, arms, fingers and legs on August 28, 2018 as a result of the following: “Machine operator pulling parts over and over . . . from standing too long & up & down.” In their Form AR-2 filed November 6, 2018, Respondents No. 1 stated that they were controverting the claim because there was “[n]o medical evidence of an injury” and because there was “[n]o employee/employer relationship.” They propounded discovery to Claimant on November 16, 2018; however, he has yet to respond to this.
